Technical Assistance:

Licensing inspector discussed training, children's records and subsidy requirements with the provider.

Comments:

An unannounced monitoring inspection was conducted today from approximately 11:40am-12:50pm. There were 2 children (8 points) in the direct care and supervision of 1 caregiver. A sample of 2 children?s records, 2 caregiver's records and 1 household member's record were reviewed. The children were observed playing on the playground. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #:	22VAC40-111-920
Description:	Based on observation and interview, the menu was not completed for one week and posted. Evidence: There was no menu posted and the provider stated she was in the food program.
Plan of Correction:	I will post it now.
Standard #:	22VAC40-111-50-A
Description:	Based on record review, the family day home did not keep a written record of children in attendance each day. Evidence: The provider could not locate the attendance for the month of March.
Plan of Correction:	Provider said she will start it today.
Standard #:	22VAC40-111-60-A
Description:	Based on interview, the provider did not maintain an up-to-date record for each child enrolled. Evidence: 1. A record for Child A was not available. 2. The provider stated she has asked the parent for the information many times.
Plan of Correction:	The provider said she would ask the parent again.
