Technical Assistance:
Please send documentation of the results of the Federal Fingerprint background check to your licensing inspector within 10 days (or immediately upon receipt). Please document steps taken to obtain the results to include emails and phone calls to follow up on receiving results as applicable.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 4/20/2020 and concluded on 4/20/2020. The licensee was contacted by telephone to initiate the inspection. There were 4 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of required documentation for enrolled children, the provider did not ensure that each enrolled child's record contained the name address and phone number for 2 designated persons to contact in case of an emergency if the parent cannot be reached.
Evidence:
Documentation for three enrolled children was reviewed.
The enrollment paperwork for child #1 contained only one of the two required designated emergency contacts.
The enrollment paperwork for child #2 contained 2 designated emergency contacts. However, there were no addresses documented for these 2 contacts.

Plan of Correction: The provider will ask the parents of child #1 and child #2 to provide the addresses for the designated emergency contacts and ensure that they are documented in their records by the end of the day tomorrow.

Standard #: 63.2-1721.1-B-2
Description: Based on a review of documentation and interview, the provider did not ensure that federal finger print criminal record searches are conducted for all adult household members.
Evidence:
The provider listed three household members on her application for license renewal which was received in the licensing office on 2/18/2020.
On 4/20/2020 when the renewal inspection was conducted, the provider stated that she had not had a federal fingerprint background check for household member #2 who had returned to residing in the home in January 2020.

Plan of Correction: The provider stated that she would begin the process of requesting the federal fingerprint background check today and would forward the results of the background check to licensing upon receipt.
