This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 4/28/2020 and concluded on 5/1/2020. The provider was contacted by telephone to initiate the inspection. There were no children present. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children's records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Standard #: 22VAC40-111-60-B Description: Based on record review, in one of two children's records reviewed, the provider did not ensure that each child's record contained all required information.
Evidence: The provider confirmed that there was not a phone number for one of two emergency contacts in the record for Child #2.
Plan of Correction: The provider will contact the parent to update the phone number.
Standard #: 22VAC40-111-70-A Description: Based on record review, in one of two children's records reviewed, the provider did not ensure that before the child's first day of attendance, parents shall be provided in writing whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia.
Evidence: The provider confirmed that there was no signed documentation in the record for Child #1 (first date of attendance 9/2/19) regarding whether or not the family day home had liability insurance.
Plan of Correction: The provider will have the parent to sign the document once normal operations resume at the family day home.
