Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 5/4/20 and concluded on 5/6/20. The provider was contacted by telephone to initiate the inspection. There were 11 children present with two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed three children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider failed to obtain a physical examination prior to attendance or within 30 days after attendance for three out of three children's records reviewed.

Evidence:

1. The provider forwarded documentation of three children's records. None of the documentation contained physical examinations. The provider stated she did not have any other information for the three children.
2. Child 1's first date of attendance was 2/3/20.
Child 2's first date of attendance was 1/2/20.
Child 3's first date of attendance was 1/2/20.

Plan of Correction: The provider will advise the parents of the three children to provide documentation of the children's physical examinations as soon as they can be obtained. In the future the provider will ensure all required paperwork is obtained in the time frame required by the standards or the child will not be allowed to return until the documentation is provided.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, the provider failed to obtain all the required information for children's records prior to the first date of attendance.

Evidence:

1. The provider forwarded documentation for three children's records. Two children's records were missing information.
2. Child 1's record did not contain the mother's work phone number and the street address for one of the emergency contacts. The start date for child 1 was 2/3/20.
3. Child 2's record did not contain the mother's work phone number.

Plan of Correction: The missing information will be obtained from the parents and added to the children's records. In the future the provider will review children's records as they are received to ensure all the required information is provided.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview, the provider failed to obtain documentation of immunizations prior to the first date of attendance.

Evidence:

The record for child 2 contained immunizations dated 1/31/20. The first date of attendance was documented as 1/2/20. The provider verified she had no other immunization records for child 2.

Plan of Correction: In the future the provider will not allow a child to attend until immunization records are provided.
