Technical Assistance:
Discussed with the provider the need for operating within the hours she is licensed and that any request to change the licensed hours needs to be submitted in writing to the licensing inspector.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was conducted on 5/12/20. The provider was contacted by telephone to initiate the inspection. There were three daycare children and two household children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, the provider's record, and one household member's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/14/20. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-30-A
Description: Based on review of attendance records and interview with the provider, the provider did not ensure compliance with the terms of the current license issued by the department. Evidence: 1. Attendance records for Child A, Child B, and Child C for May 1-11, 2020 submitted by the provider show documentation of the children being signed out of care each day between 6:30pm and 6:50pm when the current license has hours of operation listed as closing at 5:30pm. 2. Attendance records for Child A, Child B, and Child C indicate attendance on Saturday, May 2, 2020 and Saturday, May 9, 2020 while the current license is for operation Monday through Friday.

Standard #: 22VAC40-111-90-C
Description: Based on review of two children's records, the family day home did not obtain documentation of additional immunizations for a child once between the child's fourth and sixth birthday. Evidence: The most recent immunizations on record for Child B, with a date of birth of 9/14/13, were dated 9/14/15.
