Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 05/15/2020 and concluded on the same date. The provider was contacted by telephone to initiate the inspection. There were no children present at the time. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, one out of two children's records reviewed did not contain all required information.
Evidence:
1) The record for Child #2 did not contain first date of attendance and proof of the child's age and identity.
2) The Provider stated that she thought she had filled that section out.

Plan of Correction: I will contact the child's parent to request the documentation.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview, the Provider did not obtain a subsequent tuberculosis (Tb) screening every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1) The record for Caregiver #1 did not contain a current Tb screening report. The most recent Tb screening report available for review today was dated 01/19/2017.
2) The Provider stated that she did not realized it had expired.

Plan of Correction: I will get in contact with my doctor and schedule an appointment to complete the Tb screening once the State of Emergency has passed.
