Comments:
A (monitoring/renewal) inspection was initiated on 05/11/2020 and concluded on 05/14/2020. The provider was contacted by telephone to initiate the inspection. There were 7 children present and 2 staff. The inspector emailed and texted the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

An exit discussion was conducted with the provider to review the violation prior to closure of the inspection.

Violations:
Standard #: 22VAC40-111-170-A
Description: Based upon review of two caregiver records and caregiver interview, the provider has not ensured that a tuberculosis screening is obtained for each caregiver prior to hire and before coming into contact with the children.
Evidence:
1. The provider was unable to provide documentation of a tuberculosis screening for caregiver 2
2. Both the provider and caregiver 2 confirmed that caregiver 2 has been in contact with the children since August 2019 and a tuberculosis screening was not obtained.

Plan of Correction: The provider responded with the following:
Caregiver 2 has scheduled a tuberculosis screening and results will be placed in her record.
