Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 5/14 and concluded on 5/19. The provider was contacted by telephone to initiate the inspection. There were no children present and no staff other then the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance's with applicable standards or law and violations were documented on the violation notice issued to the facility.
The provider is closed, she tried to reopen but her parents are not ready to return to the childcare yet. Please call me at 703-203-8659 if you have any questions regarding this remote inspection.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on the review of two children's records it was determined that the vendor failed to get so of the required information to make the record complete.
Evidence: The provider failed to get a second emergency contact for child B.

Plan of Correction: I have asked the parents for a second emergency contact and they tell me all the people they know live in Puerto Rico, I will ask them again when they return to my daycare.

Standard #: 63.2-1720.1-A
Description: Based on the review of two staff records, it was determined that one does not have a current CPS check.
Evidence: The providers CPS check expired on 4/6/20 and she sent her request for a new search on 3/30/20. She has not received the form back yet.

Plan of Correction: I will send you the form as soon as I get it back.
