Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 5/19/20 and concluded on 5/20/20. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
On the date of inspection Child 1 did not have documentation of a physical examination by or under the direction of a physician. The child's first date of attendance is listed as: 11/18/2019.

Plan of Correction: Fixed already! The mom gave me a copy of the physical exam with the doctor signature.
