This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 21, 2020 and concluded on May 21, 2020. The provider was contacted by telephone to initiate the inspection. There were two children(7pts) children present and one staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (#) children?s records and (#) staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain documentation of additional immunizations for a child.
Evidence:
1. Child #1's first date of attendance was 07/05/2019. The last immunization documented was dated on 06/27/2019.Plan of Correction: I will ask the parent for an updated immunization record and put it in the file.
