This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/21/2020 and concluded on 05/26/2020. The provider was contacted by telephone to initiate the inspection. There were 11 children present (one residential) and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-60-B Description: Based on record reviews, the provider failed to obtain all required information for the children's records.
Evidence:
1. The inspector reviewed two children's records. One of two records reviewed (child record #2) did not have street addresses for the listed emergency contacts.Plan of Correction: The provider will contact the child's parents and obtain this information.
