54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The Code of Virginia states that ?Proof of identity means a certified copy of a birth certificate or other reliable proof of the child?s identity and age. ? The following documents are acceptable forms of reliable proof:
-Certified copy of a birth certificate
-Birth registration card
-Notification of birth (hospital, physician, or midwife record)
-Passport
-Copy of placement agreement or entrustment agreement from a child placing agency (foster care and adoption agencies)
-Record from a public school in Virginia
-Certification by a principal or his designee of a public school in the U.S. that a certified copy of the child?s birth record was previously presented
-Copy of the conferring temporary legal custody or entrustment agreement of a child to an independent foster parent.
-Child identification card issued by the Virginia Department of Motor Vehicles (DMV)

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/28/20 and concluded on 5/28/20. The provider was contacted by telephone to initiate the inspection. There were 7 children present with 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-80-A
Description: Based on record review and interview, the provider failed to obtain verification of identity within seven business days of a child's first day of attendance for one out of three children.

Evidence:

1. The record for child 2 was reviewed. The start date was 8/29/19. The provider did not obtain or observe an acceptable verification of identity.
2. The provider thought the hospital discharge papers were an acceptable form of verification of identity.

Plan of Correction: In the future only the listed acceptable forms of verification of identity will be accepted. The parent for child 2 will be advised to provide verification of identity.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview, the provider failed to obtain documentation that a child was adequately immunized before the first date of attendance.

Evidence:

1. The record for three children were reviewed. Child 2's start date was 8/29/19. The immunization record was dated 9/30/19.
2. The provider verified the child's start date and date of the immunization record.

Plan of Correction: In the future children's records will be reviewed prior to the first date of attendance and no child will be allowed to begin care until immunization records are provided.
