Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was conducted on 5/28/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and the provider's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on 6/4/2020. Standard #:	22VAC40-111-100-A
Description:	Based on record review, 1 of 2 children's records did not contain documentation of a physical examination prior to attendance or within 30 days after first day of attendance. Evidence: Child #1 (Start Date: 8/12/19) does not have a physical examination on file.
Plan of Correction:	All the forms will be brought to me by June 3, 2020.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, 1 of 2 children's records reviewed did not contain all required information. Evidence: Child #1's record was missing mother's work phone number, father's employer, father's employer address, father's work phone number and proof of child's age and identity.
Plan of Correction:	I have spoken with the parents and they will bring me all the forms I need by Wednesday, June 3, 2020.
