Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal) inspection was initiated and concluded on June 1, 2020. The provider was contacted by telephone to initiate the inspection. There were (2) children present and (1) staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s record, (1) staff record, and 1 household member record submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined a non-compliance with an applicable standard and was documented on the violation notice issued to the FDH.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review and interview, 1 of 1 staff files and 1 of 1 household member files indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent documentation on file.

Evidence -
1. On the previous date of inspection (11/07/2019), it was determined that Staff #1 and Household Member #2 both obtained TB documentation on 04/30/2018.
2. The Provider stated (during an interview conducted during the current date of inspection - 06/01/2020), that new TB documentation has not been obtained.

Plan of Correction: I will consult with my doctor to schedule in order to obtain new documentation.
