Comments:
This inspection was conducted by licensing staff using an alternate remote control, necessary due to a state of emergency declared by the Governor of Virginia.

A monitoring inspection was initiated on 05/29/2020 and concluded on 05/29/2020. The provider was contacted by video call to conduct the inspection virtually. There were 7 children present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children's records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of two children's records on 05/29/2020, the provider did not ensure that one of the records reviewed contained the required information.

Evidence: 1) The record for Child #2, enrolled on 12/10/17, did not contain the address for the second emergency contact listed. A child's record shall contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: The provider will obtain the missing address today.
