63.2 Licensure and Registration Procedures

Technical Assistance:
Technical assistance was provided regarding emergency drills. Fire drills must be completed monthly and shelter in place drills must be completed two times per year. Documentation must be maintained of all emergency drills.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was conducted on 5/26/2020. The provider was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, the provider's record, and two household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Tuesday, June 2, 2020. Violations:
Standard #: 22VAC40-111-90-A
Description: Based on record review, 1 of 2 children's records reviewed did not contain documentation that the child has been adequately immunized before the child attends the family day home.

Evidence: Child #1 (Start Date: 7/12/19) did not have an immunization record on file.

Plan of Correction: Notarized immunization declination forms (religious exemption) entered in records on 5/28.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review and staff interview, 1 of 2 adult household member files reviewed did not contain the results of a Virginia central registry search conducted within 30 days of the adult beginning to reside in the home.

Evidence: Adult Household Member #2 began residing in the family day home on 3/28/20 and does not have the results of a central registry search on file.

Plan of Correction: Request for records check mailed Thursday 5/21/20. Returned 5/28 for P.2. Notary signature and seal obtained and package re-mailed to Richmond Monday 6/1.

Description: Based on record review and staff interview, 1 of 2 adult household member files reviewed did not contain the results of a fingerprint based criminal history background check.

Evidence: Adult Household Member #2 does not have fingerprint results on file.

Plan of Correction: Appointment made 5/21 for fingerprints on Monday 6/8.

Standard #: 63.2-1721.1-B-3
Description: Based on record review and staff interview, an out of state central registry search request was not completed on 1 adult household member who has resided outside the state of Virginia within the last 5 years.

Evidence: Adult Household Member #2 began residing in the family day home on 3/28/20. The household member has resided in Maryland and California within the last 5 years as indicated on a Sworn Disclosure Statement signed 5/21/2020 and does not have an out of state central registry search request on file for either state.

Plan of Correction: Process being explored by Dogwood staff. We have procured the necessary forms and are in the process of completing them for mailing.
