Comments:

An unannounced renewal inspection was conducted on 03/04/2020 from 10:05 am to 12:05 pm. The children were observed building with blocks, cutting shamrocks, and watching television. There were four children in attendance during the inspection, totaling nine points. Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate, the inspector interacted with children. All areas utilized for child care were inspected. The provider does administer medication. The provider does provide transportation. The provider does have pets. Records were reviewed for four children, one caregiver, and two household members. Last emergency drill: 02/20/2020 Last shelter-in-place drill: 02/03/2020 There were no repeat violations found during the inspection. The Department has not yet received a complete renewal application and annual fee from the provider. A complete renewal application and fee must be received by the Department prior to expiration of the license on 04/30/2020 or the license cannot be renewed. Standard #:	22VAC40-111-60-A
Description:	Based on record review and interview, the provider did not ensure that two of five child records were up-to-date. Evidence: 1. The records of child #2 and child #4 did not contain the name and policy number of the child's medical insurance. 2. The records of child #3 and child #4 did not contain a physical/results of a health examination. 3. The provider confirmed that the policy numbers and physicals were not in the children's records.

Description:	Based on record review and interview, the provider did not ensure that a caregiver had a minimum of 16 hours of annual training. Evidence: 1. Caregiver #1 did not have record of 16 hours of annual training from 10/31/2018 to 10/31/2019 2. The provider confirmed that 16 hours of annual training had not been completed.
