63.2 Facilities and Programs

Technical Assistance:
1. COVID-19 health and safety guidance reviewed. The provider has increased her health and sanitation practices.
2. Children are required to have physical exams from the doctor within 30-days of enrollment.
3. Model forms provided: VDH physical exam, child enrollment record, information for parents, and topical permission.

Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time.

An initial inspection was initiated on 6/2/20 and concluded on 6/4/20. The provider was contacted by phone to initiate the inspection. There were no children present on 6/2/20, while there was 1 child = 4 points with the provider present on 6/3/20. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 1 child record, the provider's record, and 1 household member record submitted by the provider to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or laws, and violations were documented on the violation notice issued to the provider.

Standard #: 22VAC40-191-40-C-1-a
Description: Based on records reviewed, it was determined that the applicant/provider did not have a central registry check (CPS) completed within the last 30-days. Evidence: The provider submitted to the inspector a CPS check form, notarized on 5/23/20, that has not yet been processed.

Plan of Correction: The provider said she sent the CPS check form to the central registry on 6/3/20. The results of the check will be sent to the inspector when received.

Standard #: 22VAC40-191-40-C-1-c
Description: Based on records reviewed, it was determined that an adult living in the home did not have a central registry check (CPS) completed within the last 30-days. Evidence: The provider submitted a CPS check form for Adult #1, notarized on 5/19/20, that has not yet been processed.

Plan of Correction: The provider said she sent the CPS check form to the central registry on 6/3/20. The results of the check will be sent to the inspector when received.
