Technical Assistance:

One or more background checks were found to be missing or incomplete during today's inspection. The provider must provide documentation to the licensing inspector that the applicable background checks have been initiated within 10 business days of the inspection.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 5/28/2020 and concluded on 6/8/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed 2 children's records, the providers record and 1 household member record submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The licensing inspector has reviewed wit the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-111-100-A
Description:	Based on record review, the provider has not obtained documentation of a physical exam prior to or within 30 days after the first date of attendance for each child. Evidence: Child #1 (date of enrollment: 2/27/2017) file does not contain a physical exam.
Plan of Correction:	I will ask the mom to request the form from the doctor. I will pay the fee that the doctor charges for health forms.
Standard #:	63.2-1721.1-B-2
Description:	Based on interview, each household member over the age of 18 has not submitted to a fingerprint background check. Evidence: Household member #1 turned 18 years of age in March 2020. The provider stated that the household member did not do the fingerprint background check because of the Coronavirus.
Plan of Correction:	I will schedule an appointment for him to go.
