Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on June 12, 2020. The provider was contacted by telephone to initiate the inspection. There were (7) children present (18 points) with (2) caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) staff/household member records submitted by the family day home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined (1) non-compliance with an applicable standard and was documented on a violation notice issued to the FDH.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review, 2 of 2 staff/household members files indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent documentation on file.

Evidence -
1. The Provider stated (during an interview conducted on the date of inspection - 06/12/2020) that the most current TB documentation on file for Staff/Household Member #1 was dated - 04/24/2018.
2. The most recent TB documentation on file for Staff/Household Member #2 was dated - 06/08/2018.

Plan of Correction: An appointment shall be scheduled in order to obtain updated documentation to be placed on file.
