Comments:
The inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection initiated on June 15, 2020 and concluded on 06/16/2020. The provider was contacted by telephone to initiate the inspection. There were four children for a total of 12 points present and one caregiver. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed two children records and one caregiver record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidelines for Child Care Programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of children records, the provider failed to obtain documentation of a physical examination within 30 days after the first date of attendance.

Evidence:
1. Child #1's record did not contain documentation of a physical examination.Date of enrollment for Child #1 was 11/11/2019.
2. Child #2's physical examination was dated 01/24/2020. Date of enrollment for Child #2 was 10/28/2019.
3. The provider verified that there was no documentation of a physical for Child #1 and the date of enrollment for Child #2, and the date of the physical examination on file.

Plan of Correction: The provider contacted the parents of Child #1 to request a copy of the physical examination. In the future the provider will track the date of enrollment for children and the due date of the physical examinations.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children records, the provider failed to ensure that children records contained all required information.

Evidence:
1. Child #1's file did not contain a policy number for health insurance.
2. The provider verified that the child record did not contain the policy number for health insurance.

Plan of Correction: The provider will contact the parents of Child #1 and obtain the policy number for health insurance.

Standard #: 22VAC40-111-90-A
Description: Based on review of children records, the provider failed to obtain documentation of immunizations before the first day of attendance.

Evidence:
1. Child #2 exemption for immunizations were dated 01/21/2020. The first day of attendance for Child #2 was 10/28/2019.
2. The provider verified the first day of enrollment for Child #2, and the date the exemptions were received.

Plan of Correction: In the future the provider will ensure that immunizations are obtained before a child's first day of attendance.

Standard #: 22VAC40-111-140-B-3
Description: Based on review of provider files, the provider failed to ensure that a current cardiopulmonary resuscitation (CPR) and first aid (FA) certification were maintained.

Evidence:
1. The providers last documented certification in CPR and FA expired in October 2019.
2. The provider verified that the CPR/FA certification had expired and a new certification had not been obtained.

Plan of Correction: The provider will schedule a CPR/FA class as soon as one becomes available.

Standard #: 22VAC40-111-180-A
Description: Based on a review of provider records, the provider failed to obtain an updated tuberculosis screening every two years from the date of the last screening.

Evidence:
1. The provider's last documented tuberculosis screening was 01/16/2018. An update should have been completed in January 2020.
2. The provider verified the date of the last tuberculosis screening for Caregiver #1.

Plan of Correction: The provider will obtain a new tuberculosis screening as soon as possible.
