This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on June 22, 2020 and concluded on June 22, 2020. The provider was contacted by telephone to initiate the inspection. There were seven children present and three staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-100-A Description: Based on record review, provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1's first date of attendance was 01/06/2020. There was no documentation of a physical examination available for review despite having immunizations administered on 12/19/19 and 02/21/2020.Plan of Correction: Provider stated that the family has made an appointment with the Primary Care Physician next week, and the Physician will complete the physical form. The provider will obtain a copy of the physical and place it in the record.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #2's record did not contain the street addresses for two emergency contacts.Plan of Correction: Provider will ask the parents to complete the record with the street addresses for both emergency contacts.
