Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A modification inspection was initiated on June 18, 2020 and concluded on June 18, 2020. The director was contacted by telephone to initiate the inspection in response to license modification request for a change of location. There were no children present and 1 staff.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-111-760-A
Description: Based on observation and interview on June 18, 2020, the provider did not ensure there was a first aid kit and ice pack or cooling agent available for use in the family day home and wherever children are in care.
Evidence: 1. There was no first aid kit present.
2. The provider stated the first aid kit was not there and was at the provider's previous home.

Plan of Correction: The first aid kit and emergency medical supplies were retrieved from the previous location and are now available at the new site. The first aid kit and emergency medical supplies will remain stocked and affixed in the new location permanently.

Standard #: 22VAC40-111-770
Description: Based on observation and interview on June 18, 2020, the provider did not ensure to have a working battery-operated weather band radio.
Evidence: 1. There was no weather-band radio present.
2. The provider stated it was at her previous residence.

Plan of Correction: A weather band radio was retrieved from my previous location and is now available in the new site. The weather band radio will remain on-site permanently.

Standard #: 22VAC40-111-790
Description: Based on observation and interview on June 18, 2020, the provider did not ensure to have the required telephone numbers posted in a visible area close to the telephone.
Evidence: 1. There were no posted telephone numbers close to the telephone.
2. The provider stated the telephone numbers were still at her previous home.

Plan of Correction: The non-emergency number listing was retrieved from my previous location and is now posted in the new site. The non-emergency number listing will remain on-site permanently.

Standard #: 22VAC40-111-960-C
Description: Based on observation and interview on June 18, 2020, the provider did not ensure high chairs with safety waist and crotch straps fastened according to the manufacturer's instructions shall be used for children under 12 months of age who are not held while being fed.
Evidence: 1. There was no high chair present.
2. The provider stated the youngest child in care is 8 months old.
3. The provider stated the high chair was at the previous residence.

Plan of Correction: High chairs were retrieved from my previous location and are now on-site in the new location. The high chairs will remain on-site permanently.

Standard #: 22VAC40-111-250-A
Description: Based on observation and interview on June 18, 2020, the provider did not ensure hanging items including, but not limited to, window blinds or curtain cords, appliance cords, and ropes shall be out of the reach of children under five years of age.
Evidence: 1. There were several television cords hanging from a mounted television above the fireplace. The cords hung low enough to be within reach of children under the age of five.
2. The provider stated she knew they should be up.
3. The provider does care for children under age five.

Plan of Correction: A contractor will be on-site June 23, 2020, to cover the exposed wires and conceal the wires in the wall. He will finish up by using wire covers on any wires not enclosed in the walls. This is a permanent solution.

Standard #: 22VAC40-111-250-E
Description: Based on observation on June 18, 2020, the provider did not ensure latex gloves, balloons, and empty plastic bags large enough for a child's head to fit inside shall be inaccessible to children under five years of age.
Evidence: There was a box of gloves located under an unlocked bathroom sink used by children under age five.

Plan of Correction: Corrected on site. The latex gloves will remain in a locked cabinet.

Standard #: 22VAC40-111-280
Description: Based on observation and interview on June 18, 2020, the provider did not ensure potentially poisonous substances, materials and supplies, such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.
Evidence: 1. Located in an unlocked cabinet under the kitchen sink were Dawn dish soap, Cascade dishwasher cleaner, and Jet Dry. Each product label contained the phrase "keep out of reach of children." The Jet Dry and Dawn labels also stated "caution."
2. The provider said she didn't know those items were under the sink.

Plan of Correction: Corrected on site. The items were placed in a locked cabinet and will remain in the locked cabinet.

Standard #: 22VAC40-111-290
Description: Based on observation on June 18, 2020, the provider did not ensure sharp kitchen utensils and other sharp objects shall be inaccessible to children unless being used by the caregiver or with children under close supervision.
Evidence: There were two serrated knives located in a kitchen drawer that was accessible to children.

Plan of Correction: Corrected on site. The serrated knife was removed and placed in a high location on-site. The serrated knife will remain in the high location permanently.

Standard #: 22VAC40-111-430-A
Description: Based on observation and interview on June 18, 2020, the provider did not ensure that children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.
Evidence: 1. There are 2 stair wells with no gates installed to prevent access.
2. The provider stated the youngest child in care is 8 months old and crawling.
3. The provider stated she did not have the gates installed yet.

Plan of Correction: A gate will be professionally installed to ensure compliance. Because this is a permanent installation the stairs will be gated continually. A contractor will be on-site to complete the installation.
