This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A modification inspection was initiated on June 12, 2020 and concluded on June 12, 2020. The provider was contacted by telephone to initiate the inspection, in response to license modification request for a change of location. There was 1 child present and 1 staff.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-111-330-A Description: Based on observation on June 12, 2020, the provider did not ensure to have a landline telephone, excluding a cordless or cell phone, available, operable, and accessible.
Evidence: When asked to see the landline phone, the provider stated it was in a drawer, not hooked-up, and that she couldn't get to it and didn't know where it was.Plan of Correction: I have replaced my cordless phone with the landline phone.
