Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on July 14, 2020 and concluded on July 14, 2020. The provider was contacted by telephone to initiate the inspection. There were nine children present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain the required information.
Evidence:
1. Child #1 and Child #2 files did not contain the name and policy number of the child's medical insurance.

Plan of Correction: Provider will obtain the name and policy number of the child's medical insurance from the parents and document each child's record.

Standard #: 22VAC40-111-80-A
Description: Based on record review, provider did not obtain proof of age and identity from the parent within seven days of the child's first day of attendance.
Evidence:
1. Child #2's first date of attendance was 03/06/2020. Provider did not have documentation of verification of identity and age in the record.

Plan of Correction: The provider will ask the parent to provide verification of identity and age of the child and document the information in the record.
