This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A Monitoring inspection was initiated on date7/9/2020 and concluded on 7/15/2020. The Provider was contacted by telephone to initiate the inspection at 10:55am. There were 3 children present (10 points) and 2 staff. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed 2 children's records, 2 staff records, and 4 household member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-111-80-A Description: Based on a review of children's records and Provider interview, 1 out of 2 records did not contain verification of the identity and age of the child within seven business days of the child's first day of attendance.
Evidence:
1. Child #1 (start date: 8/25/2019) did not have documentation on file indicating that the Provider has verified the age and identity of the child.
2. The Provider stated that Child #1 does not have a birth certificate or any verification of age and identity on file.Plan of Correction: I will ask the parents today to bring it.
