Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 7/15/2020 and concluded on 7/15/2020. The provider was contacted by telephone to initiate the inspection. There were 9 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with a
applicable standards or law and the violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review of two children's records, the provider failed to have all required information in each child's file. Evidence: Child 1 (date of enrollment 5/1/2020) did not have the name, address, and phone number of two emergency contacts on file.

Plan of Correction: Provider will request this information from the parent.

Standard #: 22VAC40-111-70-A
Description: Based on review of two children's files, the provider failed to have documentation that each parent had been provided in writing the provisions of the family day home's emergency preparedness and response plan. Evidence: there was no documentation that Child 1 (date of enrollment 5/1/2020) had received the VDSS model form "Provisions of the Emergency Preparedness and Response Plan" or an equivalent.

Plan of Correction: Provider will get signed verification that each parent has the "Provisions of the Emergency Preparedness and Response Plan" form.
