Comments:
A complaint was received in the licensing office on July 10, 2020 regarding child abuse. On July 14, 2020 a virtual/audio investigation in conjunction with an outside agency was conducted with the providers. Based upon interviews, the preponderance of evidence does not support the allegation. Violations cited today were found during the course of the investigation. Complaint related: No
Description: Based on interview with provider, the provider did not record a injury sustained by a child while at the family day home.
Evidence:
There is no record of the injury child #1 sustained while at the family day home.
Upon arrival parent of child #1 was informed by provider of the injury.

Plan of Correction: From now on I will make sure to write a injury record for future marks.

Complaint related: No
Description: Based on interview with provider, the provider did not have the awareness of and responsibility for each child in care, including being near enough to intervene if needed.
Evidence:
While in the care of the provider child #1 sustained an injury on their leg.
Provider assisted child #1 with potty training 3-4 times before nap time.
Provider assisted child #1 putting on a pull up before nap time.
Provider assisted child #1 removing the pull up after nap time.
Child #1 had a potty accident, provider placed child #1 on the diaper changing table, provider noticed injury to child #1's leg at that time.
Provider had no knowledge of how the child received the injury.

Plan of Correction: From now on we will make sure to check children once in a while in case if they have any injury or mark.
