Technical Assistance:

1. Reviewed diaper changing procedures. The inspector will provide a procedure flyer for posting in the home. 2. The inspector will send the provider a current copy of a Sworn Disclosure Statement, which she be used when updating this form in staff/adult records. 3. The provider requested the age range of children allowed to be in care be expanded, and was advised to put this request in writing to the inspector for consideration.

Comments:

This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the home was not conducted at this time. An announced monitoring inspection was conducted today, from 1:00-1:57pm, by a remote virtual process that allowed the inspector and licensing administrator to observe the physical environment and care of children. There were 6 children = 16 points with the provider, and assistants #1 + #2. Ratios and supervision were in compliance during the inspection. The inspector reviewed 2 child records, 1 caregiver record, and 2 household member records to verify corrective actions taken from the May 2020 inspection. Areas of non-compliance with applicable standards or laws, and violations were documented on the violation notice issued to the provider. Standard #:	22VAC40-111-490
Description:	Based on observations made and interview conducted, it was determined that the indoor slide over 18" in height did not have a resilient surface underneath. Evidence: The slide/climbing equipment, used by the children for active play during the hot weather, had carpet underneath of it.
Plan of Correction:	The provider said she has rubber mat tiles that she will put under the climbing unit/slide.
Standard #:	22VAC40-111-690-D
Description:	Based on interview conducted, it was determined that the diaper changing surface has not been cleaned and sanitized after each use. Evidence: The provider stated that the diaper pad has not been cleaned, but has been sanitized after each use.
Plan of Correction:	The provider said that they will clean the diaper pad with soap + water before sanitizing it after each diaper change.

Description:	Based on observations made, it was determined that the findings from the most recent inspection were not posted in the home. Evidence: The inspection report, including violation notice, from the inspection dated 5/7/20-5/8/20, was not posted.
Plan of Correction:	The provider will put this inspection report on the bulletin board in the daycare.
