Technical Assistance:
Provided consultation on standard 630: Caregivers shall use positive methods of discipline. Discipline shall be constructive in nature. When time out is used, it shall be used sparingly and shall not exceed one minute for each year of the child?s age; time out shall not be used with infants or toddlers (0-24 months.)

Provided consultation on standard 900: To assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery such as whole hot dogs sliced into rounds, nuts, seeds raisins, uncut grapes, uncut raw carrots, peanuts, chunks of peanut butter, hard candy, and popcorn shall not be served to children under four years of age unless the food is prepared before serving in a manner that will reduce the risk of choking.

Provided consultation on standard 960B: Infants who cannot hold their own bottles shall be picked up and held for bottle feeding.

Provided consultation on COVID-19 Guidance for programs that remain open: Infection Control and Sanitation Practices During COVID-19: Implement robust policies and procedures for handwashing and cleaning and sanitizing frequently touched surfaces.
"Cleaned" means treated in such a way as to remove dirt and debris by scrubbing and washing with soap and water or detergent solution and rinsing with water; "Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of bleach water prepared fresh daily. The surface of the item is sprayed or dipped in the sanitizing solution and then allowed to air dry.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was conducted on 7/27/2020. An exit interview was conducted on 7/31/2020. The provider was contacted by telephone to initiate the remote virtual inspection. There were no children present at the time of the inspection. The Inspector reviewed two provider/household member records, the menu, the emergency preparedness plan, the first aid kit, battery operated flashlight and weatherband radio. Toys, equipment, and materials were observed. Three cribs meet the US CPSC standards. The bathroom and diapering area were observed. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, and an interview with the provider, not all required items were in the first aid kit. Evidence: There was no first aid instructional manual, tweezers, guaze pads or triangular bandages in the first aid kit.

Plan of Correction: All missing items have been purchased.

Standard #: 22VAC40-111-770
Description: Based on observation, and an interview with the provider, not all required emergency items were available in the home. Evidence: there was no working battery-operated flashlight, portable battery-operated weather band radio, or extra batteries in the home.

Plan of Correction: Items were purchased.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all furnishings of the family day home were maintained in a safe condition. Evidence: one of the three cribs in the daycare area had peeling paint on the railings.

Plan of Correction: Provider has purchased paint and will repaint crib. Crib will not be used until fixed.

Standard #: 22VAC40-111-240-B
Description: Based on observation, and review of documentation, the provider failed to ensure no equipment is used if recalled by the U.S. Consumer Product Safety Commission (CPSC). Evidence: there was a Fisher Price Rock n Play sleeper observed in the daycare area. This product was recalled in April of 2019.

Plan of Correction: The item was removed from the day care area.

Standard #: 22VAC40-111-280
Description: Based on observation, not all potentially poisonous substances were kept inaccessible to children. Evidence: A cannister of Clorox wipes was observed on the toilet in the bathroom.

Plan of Correction: The item was moved to a locked cabinet in the bathroom.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider failed to ensure that no soft bedding is used around infants. Evidence: there was a blanket with a stuffed animal head inside each of the three cribs in the daycare area.

Plan of Correction: The blankets were removed from the cribs during the inspection.
