Comments:
This Inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 08/24/2020 and concluded on 08/24/2020. The provider was contacted by phone to initiate the inspection. There were 6 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 of staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-111-60-B
Description: Based on a review of records, one child's record is incomplete.
Evidence:
1 of 2 files reviewed was missing documentation of a second emergency contact's address. (child 1)

Plan of Correction: I will ask the parent for the information this afternoon.

Standard #: 22VAC40-111-70-A
Description: Based on a review of records, one child's record does not contain written acknowledgment of all required policies and procedures.
Evidence:
1 of 2 records reviewed did not contain written acknowledgement of the notice of liability insurance declaration.

Plan of Correction: I will have the parents sign the form tonight.
