This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 08/20/2020 and concluded on 08/25/2020. The provider or in-charge person was contacted by telephone to initiate the inspection. There were 4 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-111-60-B Description: Based on record review, children's files did not contain all required information.
Evidence:
1) The records for Child #1 and Child #2 did not contain documentation of having received parent's signed acknowledgement of the receipt of the required information (Info for parents form) and results of the health examination for the child.Plan of Correction: The provider shall provide the information to the parents and include documentation in the children's records.
Standard #: 22VAC40-111-70-A Description: Based on record review, parents were not always provided in writing with all required information.
Evidence:
1) The record for Child #2 did not contain signed documentation as to whether or not there is liability insurance on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia. A liability declaration form was located in the record but it was not signed by the parents.Plan of Correction: The provider shall obtain the parents' signature and include the documentation in the child's record.
