Technical Assistance:
Documentation still needed to complete the Initial Application (outside of this items listed on issued violation notice: ID from the Provider; a letter clarifying that the FDH will operate on a 24 hour period, Monday through Saturday and that there is no fictitious name. Due to the hours of operation, the hours for overnight care were also reviewed.

Discussion that the by right number placed on the home by Fairfax County Zoning was for a capacity of 7; the Provider was encouraged to seek out the special exemption process (through zoning) to see if it is possible to increase that number.

Comments:
The following inspection took place between the hours of 10 am through 11:30 am. Today, (due to the pandemic) I met with the Provider through a virtual portal to the Family Day Home (FDH) located in Falls Church, Virginia. At the start of this announced initial inspection there were no children in care with 1 caregiver. A total of 1 staff file, and 1 household member file was reviewed. The FDH has an abundance of toys and equipment for the children to enjoy. There was a total of (2) cribs located on site that meet federal guidelines. In addition, the FDH offers a large backyard with various age appropriate playground equipment.

Violations:
Standard #: 22VAC40-111-790
Description: Based on observation, not all of the required emergency telephone numbers were posted at the family day home.

Evidence -
1. On the date of inspection (08/26/2020), the responsible person for emergency backup care (as required in 22 VAC 40-111.800.A.3) was not posted with 911 under emergency numbers.
2. The regional poison control center was also not posted with the emergency telephone numbers.

Plan of Correction: All applicable emergency telephone numbers shall be posted.

Standard #: 22VAC40-111-800-A
Description: Based on observation, the family day home (FDH) does not have a completed written emergency preparedness and response plan (EP&RP).

Evidence -
1. On the date of inspection (08/26/2020), the Provider has elected to use the VDSS model form of the 7 page EP&RP. While pages 1 through 5 were complete, the plan was missing pages 6 and 7.
2. The EP&RP was observed on file in a binder at the FDH.

Plan of Correction: The missing pages shall be found and added to the EP&RP.

Standard #: 22VAC40-111-920
Description: Based on observation, the menus posted did not include all of the required information.

Evidence -
1. On the date of inspection (08/26/2020), none of the menus posted included a date.
2. While the menus were listed on a weekly bases, they were not tied to an actual date.

Plan of Correction: Menus shall be dated.

Standard #: 22VAC40-111-170-A
Description: Based on review, a Household file indicated that an adult household member (prior to coming into contact with children) a current Report of Tuberculosis Screening (TB) form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form) was not available.

Evidence -
1. On the date of inspection (08/26/2020), TB documentation for Household Member #1 was not available for review.
2. On the date of inspection Staff #1 confirmed that Household Member #1 was a resident. Household Member #1 was also listed on page 3 of the Initial Application as a resident.

Plan of Correction: TB documentation shall be obtained for Household Member #1, shared with the Licensing Inspector, and placed on file at the FDH.

Standard #: 22VAC40-111-240-A
Description: Based on observation, an area inside the family day home FDH) was not maintained in a safe condition.

Evidence -
1. On the date of inspection (08/26/2020), in the front lobby part of the FDH, under the desk, a large tangle of electric cords were visible.
2. These cords would be accessible to the children in care and could pose a danger of entanglement.

Plan of Correction: These cords shall be secured.

Standard #: 22VAC40-111-320-B
Description: Based on observation and interview, child-resistant protective covers larger than 1-1/4 inches in diameter were not installed on all unused electrical outlets accessible to children under five years of age.

Evidence -
1. On the date of inspection (08/26/2020), outlet covers were missing from all wall outlets.
2. The Provider confirmed this during an interview conducted on the date of inspection.

Plan of Correction: Covers shall be installed in all outlets in order to prevent access by the children in care.

Standard #: 22VAC40-111-430-A
Description: Based on observation and interview, children under two years of age would have access to stairs.

Evidence -
1. On the date of inspection (08/26/2020), when the children are traveling through the kitchen in order to use the backdoor of the family day home leading to the backyard play area they pass the staircase leading downstairs to the household living space.
2. The Provider stated (during an interview conducted on the date of inspection) that the children would pass this area while on their way to go outside.

Plan of Correction: A gate shall be placed between the two walls leading to the staircase in order to block its access by the children in care.

Standard #: 22VAC40-111-690-D
Description: Based on review and interview, there was not a nonabsorbent and washable diapering surface available at the family day home (FDH).

Evidence -
1. On the date of inspection (08/26/2020) a diapering surface for changing was not available. The applicant has requested the license to have an age range from 6 weeks through 12 years.
2. The Provider stated (during an interview conducted on the date of inspection) that a changing table was on order and is expected to be delivered on 08/28/2020.

Plan of Correction: A changing table has been already ordered and delivery is expected at the end of this week.

Standard #: 22VAC40-191-40-C-1-c
Description: Based on review, a file of an adult living in the home of the applicant indicated that not all of the required documentation was on file upon application for licensure.

Evidence -
1. On the date of inspection (08/26/2020), the following documentation was not available for review for Household Member #1: Sworn statement or affirmation (SDS), search of central registry (CPS), and criminal history record check (CRC).
2. On the date of inspection Staff #1 confirmed that Household Member #1 was a resident. Household Member #1 was also listed on page 3 of the Initial Application as a resident.

Plan of Correction: All applicable documentation shall be obtained, shared with the Licensing Inspector, and placed on file at the Family Day Home.
