Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on August 28, 2020. The provider was contacted by telephone to initiate the inspection. There were 9 children (23 points) and 2 caregivers. The inspector reviewed 2 children's records and two caregivers records submitted by the facility to ensure the documentation was complete.

Violations:
Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to obtain documentation of immunizations by the first day of attendance.

Evidence:
Child #2' record does not contain documentation of immunizations until 3/8/2017, the child's first date of attendance was 02/02/2017.

Plan of Correction: I will obtain proof of immunizations of new children on the first day of attendance.
