Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 8/19/2020 and concluded on 8/19/20. The provider was contacted by telephone to initiate the inspection. There were 5 children present and 1 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility. Description: Based on record review and interview, a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction was not obtained for a child with a diagnosed food allergy.

Evidence:
Child#2 has been diagnosed with a food allergy, their record did not include a written allergy care plan.

Plan of Correction: Required forms have been given to the parent to take to the doctors.
