Technical Assistance:
Reviewed new background requirements, effective 7/1/2020, for staff who are living or have lived outside of the state of VA in the past 5 years.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 09/24/2020 and concluded on 09/24/2020. The provider was contacted by telephone to initiate the inspection. There were 24children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and provider interview, the record for 1 of 2 children did not contain all required information.

Evidence:
1. The record for Child #1 (start date 8/20/18) did not contain results of the child's health examination.
2. Provider reported the document had been obtained previously, but she had misplaced the document in the child's file.

Plan of Correction: I will get a copy or have parents get a new one filled out when the child goes for their next yearly check up.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the family day home did not obtain sworn statements of affirmation and search of central registry before five years since the dates of the last sworn statement or affirmation and most recent central registry finding for 2 of 2 staff.

Evidence:
1. The most recent sworn statement for Staff #1 and Staff #2 were dated 9/11/15.
2. The most recent central registry finding for Staff #1 and Staff #2 were dated 9/24/15.
3. Provider stated updated documents had not been completed.

Plan of Correction: Staff will complete new Sworn Statements today. The provider will provide documentation that the central registry request for Staff #1 and Staff #2 were sent within 10 business days from the date of this inspection.
