This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated and concluded on 10/1/20. The provider was contacted by telephone to initiate the inspection. There were 2 children present and 3 staff. The inspector emailed the provider a list of items required to complete the inspection. Staff and children's records were reviewed virtually. The Inspector reviewed 2 children?s records, 3 staff and 2 household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. For any questions related to this inspection, please contact me at 571-835-4718 or firstname.lastname@example.org.
Standard #: 22VAC40-111-30-A Description: Based on staff interview, the provider did not ensure compliance with the terms of the current license issued by the department.
1. The family day home license is approved for ages 3 months through 5 years.
2. During the inspection, the provider stated that she extended the age range up to 7 years old, and Child # 1 (6 years
old) and Child # 2 (7 years old) started attending the family day home on 9/21/20.
3. The terms of the license related to age range were not modified before the two older children started care.
Plan of Correction: I mailed in the request to increase the age range with renewal application, these children will only attend on Mondays and only during the state of emergency.
