Technical Assistance:
The Licensing Inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Discussion of the modification tied to this UMI. The Provider submitted a License Modification Request Facility Change of Location form. The new address for the license shall read as: 3920 Kernstown Court, Fairfax, Virginia 22033. It should be noted that this was the previous address for this Family Day Home (FDH). They are now moving back to their former location. The zoning approval form for the former (now new again) address is still on file and will be used as the "by right" number; setting the capacity at 5 children. There have been no structural changes to the floor plan, and both homes are identical in design to the other, located in the same neighborhood. This modification will take effect on November 1, 2020.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 10/01/2020. The Provider was contacted by telephone to initiate the inspection. There were (3) children present (5 points), with (1) caregiver. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records, (1) household record, and (1) staff record submitted by the Family Day Home to ensure documentation was complete.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were cited.
