Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 10/29/2020 and concluded on the same date. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record and 4 household member records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review and interview, the provider did not practice monthly evacuation drills with children.

Evidence: The last evacuation drill was conducted in July 2020.

Plan of Correction: I will practice drills monthly.

Standard #: 22VAC40-111-920
Description: Based on record review, the menu was not dated for the current one week period.

Evidence: The menu presented did not have dates written on it.

Plan of Correction: I will make sure my menus are dated.

Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Evidence: Child 1 does not have documentation of a physical and their start date was 12/2019.

Plan of Correction: I will ask the parents for the documentation.

Description: Based on record review and interview with the provider, children's records did not contain all the information needed.

Evidence: Child 2 is missing a second emergency contact.

Plan of Correction: I will ask parents for the information.

Standard #: 22VAC40-111-90-C
Description: Based on record review and interview with the provider, the provider did not obtain documentation of additional immunizations for a child.

Evidence: Child 1 has an immunization record dated 12/2019 and has no other updates.

Plan of Correction: I will ask the parent for the updated records.

Standard #: 22VAC40-111-480-D
Description: Based on observation, equipment used by children was not used in accordance with the manufacturer's instructions.

Evidence:
1. Child 2 was observed sitting in a bouncy seat wrapped in a blanket without the safety strap buckled around the child.
2. Child 2 was observed unbuckled in a bouncy seat. The bouncy seat with the child was inside a crib with the mattress at the highest level.

Plan of Correction: The child was buckled immediately. The bouncy seat was taken out of the crib placed on the floor.
