Standard 830: Emergency response drills - the emergency evacuation procedures shall be practiced monthly; shelter-in-place drills shall be practiced a minimum of twice per year.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 11/9/2020. The provider was contacted by telephone and a virtual inspection was conducted. There were three children present, ages 12 months to three years of age, for a total of 11 points with the provider. The Inspector reviewed three children's records, the emergency drill log, meals and snacks, hand washing, enhanced cleaning and sanitizing of toys and surfaces, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review of three children's records, it was determined that not all records contained all required information. Evidence: the records for child #2 and child #3 did not have the name, address, or phone number for a second emergency contact.

Plan of Correction: Provider stated that she will obtain the missing information from the parents.

Standard #: 22VAC40-111-70-A
Description: Based on review of three children's records, the provider failed to have documentation that all policies were provided to parents in writing before the first day of attendance. Evidence: The records for child #1, child #2, and child #3 did not have signed documentation that parents had been given VDSS model forms "Information for Parents," ?Policy on Administration of Medication? or ?Liability Insurance Declaration.?

Plan of Correction: Provider stated she will obtain written documentation that parents have been provided the forms.
