Discussed shelter-in-place drills and the documentation required for household members 18 years of age or older.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 10/26/2020 and concluded on 10/29/2020. The provider was contacted by telephone to initiate the inspection. There were 2 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff/household members records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-111-60-B Description: Based on record review, children's files did not contain all required information.
Evidence:
1) The records for Child #1 and Child #2 did the work phone number for one of the custodial parents.Plan of Correction: The provider shall contact the parents to submit the required information.
Standard #: 22VAC40-111-90-C Description: Based on record review, it was determined that the provider did not ensure to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1) The record for Child #2 (dob 01/19/2019) did not contain a current immunization report. The most recent report in the record for Child #2 was dated 07/19/2019.Plan of Correction: The provider shall contact the parents to submit the required information.
