Discussed the requirement for the Provider to document the name of the person who saw the child's proof of id and the date when it was seen.
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 11/16/2020 and concluded on 11/17/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 4 staff/household records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (Tb) Screening form every two years from the date of the first screening.
1) The records for Staff #1 and #2 and the record for Household Members #1 and #2 did not contain a current Tb screening. The most recent Tb screenings available for review were dated: 05/09/2018, 05/09/2018, 05/16/2018 and 05/09/2018 respectively.
Plan of Correction: The provider shall obtain a TB screening result for each Staff and Household member.
