Technical Assistance:
The Licensing Inspector has reviewed with the director/provider COVID-19 Essential Guidance for Child Care programs.

Discussion of new requirements for staff who have lived outside of the Commonwealth of Virginia within the past 5 years. For staff hired prior to 07/01/2020, the Provider has until 01/01/2021 to obtain this additional documentation.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 11/18/2020. The Provider was contacted by telephone to initiate the inspection. There were (5) children present (16 points) with (2) staff. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) staff records submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined a non-compliance with an applicable standard and was documented on the violation notice issued to the FDH.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 2 children files indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence -
1. The Provider stated (during an interview conducted on the date of inspection - 11/18/2020) that Child #1 did not have documentation on file of a physical examination.
2. The start date for Child #1 was listed as 08/31/2020.

Plan of Correction: The parent of Child #1 shall be contacted, the documentation of a physical examination obtained, and then placed on file within the Family Day Home.
