Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on November 17, 2020 and concluded on November 18, 2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of caregivers records, one record does not contain the required documentation.
Evidence:
Caregiver #1 does not have documentation of a current tuberculosis (TB) screening form. Current TB screening form expired 4/20.

Plan of Correction: A new TB screening will be completed.

Standard #: 22VAC40-191-60-B
Description: Based on review of caregivers records, one record does not contain the required documentation.
Evidence:
Caregiver #1 does not have documentation of a current signed Sworn Statement. Statement in record expired 4/20.

Plan of Correction: I will sign a new statement
