An out of state search of the sex offender registry must be completed for all staff/household residents (Family Day Homes) who have resided outside of the state of Virginia within the past 5 years. In addition, an out of state criminal history record search must be completed for staff/household residents who have resided in any non-NFF states within the past 5 years.
In response to the national health emergency, the provider has been closed since March 16, 2020 and plans to stay closed until further notice. The provider will notify the Inspector when she determines a reopening date.
The license cannot be renewed until the provider submits a fully completed Renewal Application and Staff Information Sheet.
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated and concluded on 11/19/2020. The provider was contacted by telephone to initiate the inspection. There were 0 children present and 0 caregiver staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 adult records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Standard #: 22VAC40-111-330-A Description: Based on interviews with the provider, it was determined that a landline telephone, excluding a cordless or cell phone, is not available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate.
The provider indicated that she does not have landline telephone service.
Plan of Correction: I will add landline service and send the Inspector my home phone number.
