Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 11/18/2020 and concluded on 11/19/2020. The provider was contacted by telephone to initiate the inspection. There were 4 children present (9 points) and 1 caregiver. The Inspector reviewed 2 children?s records and 2 caregiver/household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of of a physical examination by or under the direction of a physician within 30 days after the first day of attendance for one child.

Evidence:
Child #1 began attending the family day home on 12/12/16. There was no documentation of a physical examination for Child #1 available for review during inspection.

Plan of Correction: They are scheduled for a physical next month. I will ask the parents for a copy.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 2 of 2 children's parents were not provided in writing all required information before the child's first date of attendance.

Evidence:
1. The records for Child #1 (start date 12/12/16) and Child #2 (start date 10/07/19) did not contain provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: I will give the parents the form to get signed.

Standard #: 22VAC40-111-80-C
Description: Based on record review, 1 of 2 children's records reviewed did not contain documentation of proof of identification.

Evidence:
1. The record for Child #1 (start date 12/12/16) was missing the method of verification of the child's age and identity, and documentation of date and person verifying original documentation.
2. Provider stated she believed the information had been recorded on an older form, but was unable to locate the form.

Plan of Correction: The parent is bringing me proof of id tomorrow.

Standard #: 22VAC40-111-460-C
Description: Based on record review, rabies vaccinations for one pet were not current.

Evidence:
1. Rabies vaccinations for the provider's pet expired on 9/03/2020.

Plan of Correction: An appointment was scheduled for 11/23/2020.
