Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on November 30, 2020 and concluded on November 30, 2020. The provider was contacted by telephone to initiate the inspection. There were six children(11pts) present and three staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, three household records, and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not annually review with the parent the emergency contact information to ensure the information is correct.

Evidence:
Child #2's record (First date of attendance-04/22/2019) did not have a signature indicating a one year review.

Plan of Correction: The provider will have the parent to review the emergency contact information and sign the document to indicate that the information is up to date and correct.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain required information.

Evidence:
Child #2's record did not contain documentation of name and policy number of the child's medical insurance.

Plan of Correction: The provider will obtain the name of the insurance and policy number and update the record.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided in writing whether there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia prior to the child's first day.

Evidence:
Child #1's (First date of attendance-11-16-2020) and Child #2's(First date of attendance-08/05/2019) record did not have signed documentation of receipt of Liability Insurance prior to the child's first day of care.

Plan of Correction: The provider will give the information to the parents and place the documents in each record.
