This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 11/13/2020 and concluded on 11/13/2020. The provider was contacted by telephone to initiate the inspection. There were 6 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records and 1 household member record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-730-D Description: Based on observation, it was determined that one child's prescription medication was not locked.
Evidence:
Child#2's prescription medication was stored on an upper level shelf inside an unlocked container. The medication was out of children's reach, but it was not locked.Plan of Correction: Corrected on the same day. The medication is locked.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records did not all contain required information.
Evidence:
Child#1's record is missing the mother's work phone number, the child's medical insurance policy number and the phone numbers for both emergency contacts.
Child#2's record is missing the child's medical insurance policy number.Plan of Correction: It has been corrected.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain for each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the last screening or more frequently as recommended by a physician or the local health department.
Evidence:
HH#2's record contains a tuberculosis screening/test result that was last updated on 7/25/18.Plan of Correction: The household member will make an appointment to update their TB test.
