Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 11/13/2020 and concluded on 11/13/2020. The provider was contacted by telephone to initiate the inspection. There were 6 children present and 2 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-170-B
Description: Based on record review and interview, the provider did not obtain a written allergy care plan for each child with a diagnosed food allergy to include actions to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
Child#2's record does not include a written allergy care plan. The child's record indicates a diagnosed food allergy and the child has emergency medication on-site.

Plan of Correction: The parent will print the form to take it to the doctor.
