Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 11/17/2020 and concluded on 12/02/2020. The provider was contacted by telephone to initiate the inspection. There were 4 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, it was determined that the provider did not ensure to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #2 (start date 09/03/2019) did not contain documentation of a physical examination. In the record for Child #1, although it did contain documentation of a physical examination, it was missing the parent's signature on the first page of the CoVA form.

Plan of Correction: I will contact the parents immediately to provide the information.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's files did not contain all required information
Evidence:
1) The record for Child #1 did not contain the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: I will contact the parents immediately to provide the information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, it was determined that parents were not provided in writing will all required information before the child's first day of attendance.
Evidence:
1) The record for Child #1 was missing the first page of the Policies for the administration of medications form.

Plan of Correction: I will contact the parents immediately to provide the information.

Standard #: 22VAC40-111-90-A
Description: Based on record review, it was determined that the provider did not ensure to obtain documentation that the child has been adequately immunized.
Evidence:
1) The record for Child 21 (start date 09/03/2019) did not contain documentation that the child has been adequately immunized.

Plan of Correction: I will contact the parents immediately to provide the information.

Standard #: 22VAC40-111-180-A
Description: Based on record review, it was determined that the provider did not obtain a current Report of Tuberculosis (Tb) Screening form, every two years.
Evidence:
1) The record for Staff #1 did not contain a current Tb screening report. The most recent Tb screening report was dated 01/19/2017.

Plan of Correction: I will get a new Tb screening once the State of Emergency has passed.
